مناقشات علمية، توصيات، تجارب، وأكثر من ٢٠٠٠ بحث، و٧٠٠٠ مشارك، ساهمت معاً فى تشكيل فعاليات مؤتمر الجمعية الأمريكية لدراسات أمراض الكبد هذا العام، والذى عُقد فى مدينة بوسطن الأمريكية فى الفترة بين ٣١ أكتوبر و٣ نوفمبر.

ولم يخل المؤتمر من مشاركة العشرات من كبار أطباء وأساتذة الكبد فى مصر والعالم العربى، والذين ناقشوا الشأن المصرى ومدى نمو معدلات الإصابة بأمراض الكبد فى مصر، والجدل الدائر مؤخراً حول قرار هيئة التأمين الصحى باستخدام مستحضر مصرى لعلاج فيروس «سى»، وتعددت المناقشات حوله ما بين تشكيك فى فاعليته، وتحفظ على إبداء الآراء حول نتائجه.

«المصرى اليوم» رصدت مناقشات المتخصصين المصريين والأجانب حول فاعلية العقار، واستطلعت آراء الأطراف المختلفة.

المناقشات حول الأدوية المخصصة لعلاج فيروس الالتهاب الكبدى «سى» تناولت قرار هيئة التأمين الصحى تقديم عقار مصرى لعلاج مرضى فيروس «سى»، ومدى فاعليته، وفى الوقت الذى أكد فيه أطباء وأساتذة كبد فاعلية العقار المصرى وذكر بعضهم أنهم استقبلوا فى عياداتهم مرضى خضعوا للعلاج له لمدة عام ثم تعرضوا لانتكاسة، أشار آخرون إلى أن العقار يحتاج ما بين سنتين وأكثر للحصول على النتائج النهائية والدلائل الإحصائية لمدى فاعليته. ومجموعة أخرى أكدت أنه لا يقل بأى حال من الأحوال عن المثائل الأجنبية.

الدكتور طاهر حسن الزناتى، أستاذ الأمراض الباطنية والجهاز الهضمى والكبد فى كلية طب قصر العينى، استعرض تجربته مع العقار المصرى لعلاج فيروس «سى» قائلاً: «لقد جربته بالفعل، ويمكننى القول بأنه فعال فى العلاج المستمر، ويشير إلى وجود استجابة، ولكن تبقى المشكلة فى أن معدلات الانتكاسة عالية بين مستخدميه، أى أنه بعد انتهاء فترة العلاج به يرتد المرض بنسبة تصل إلى ٤٠%، علماً بأن مشكلة الانتكاسة غير مقصورة على الدواء المصرى وتحدث مع جميع الإنترفيرونات وتتراوح بين ٥٠ و٦٠٪ وفقاً للدراسات العالمية على الأدوية المثيلة الأجنبية «مجلة الكبد العالمية لعام ٢٠٠٩» وبالتالى فإن نسبة نجاح العلاج فى حدود ٥٠٪ تعتبر هى النسبة المعقولة للعلاج بالإنترفين والريبافيرين معا حسب آخر الدراسات العلمية المحلية والعالمية وهذا الكلام على لسان أ.د. جمال عصمت.

علاجات فيروس «سى» فى مصر كانت محور اللقاء الذى جمع كلاً من الدكتور جمال شيحة، أستاذ الكبد فى جامعة المنصورة، رئيس جمعية رعاية مرضى الكبد، وأكسيم كاوتس، رئيس الجمعية الألمانية للحماية من الالتهاب الكبدى، ممثل أوروبا فى التحالف العالمى للالتهاب الكبدى، حيث استعرض شيحة الأدوية المستخدمة فى علاج مرضى فيروس «سى» فى مصر، وأشار إلى الجدل الذى أُثير مؤخراً حول تعاقد الهيئة العامة للتأمين الصحى مع شركة مصرية بالقطاع الخاص «راين مينا فارم»، لتوريد عقار رايفينور ريتارد، لعلاج مرضى فيروس «سى» من المؤمن عليهم، ومدى فاعلية العقار من عدمه.

«شيحة» قال لـ«كاوتس»: «إننا نسعد عندما نسمع بإنتاج دواء مصرى، أو بوجود دواء أقل فى السعر من الأدوية التى تنتجها الشركات العالمية، لكن يجب أن تكون أدوية فعالة لأن الأمر يتعلق بصحة المواطنين»، مستنداً فى ذلك إلى دراسة أُعدت فى معهد الكبد فى المنصورة، وأخرى فى جامعة عين شمس، وأضاف أن الهيئة العامة للتأمين الصحى استندت فى قرارها بتوريد العقار على دراسة واحدة فقط تؤكد وجود نتائج إيجابية له، مشيراً إلى أن الدراسة «مشكوك فيها» على حد تعبيره، وأن الهيئة فضلت العقار لانخفاض سعره ولأنه مسجل فى وزارة الصحة.

الدراسة التى يعنيها د. جمال شيحة هى دراسة قام بها اثنان من كبار المتخصصين فى علاج أمراض الكبد وهما الأستاذ الدكتور جمال عصمت- وهو الذى يعتبر رائداً فى التجارب الإكلينيكية للإنترفيرون فى مصر والعالم ورئيس الاتحاد العالمى للكبد- بالاشتراك مع العالم الأستاذ الدكتور شريف عبدالفتاح.. ونشرت الدراسة فى مجلة عالمية وكان نتيجتها أن نسبة نجاح العلاج بالدواء المصرى وهى ما بين ٥٠٪-٥٥٪ تقع فى نفس الحدود المنشورة للأدوية الأجنبية بالنسبة لنوع الفيروس المنتشر فى مصر وهو من النوع الجينى ٤.

وتأتى هذه النتائج متماشية مع آراء بعض الأساتذة الكبار فى مجال الكبد ومعهم الأستاذ الدكتور سراج زكريا أستاذ قصر العينى المشهور الذى وصف الدواء المصرى بأنه منافس جيد، وكذلك الدكتور محسن سلامة أستاذ الكبد بمعهد الكبد بالمنوفية الذى أكد أن الدواء المصرى ليس فقط بنفس فاعلية الأجنبى لكن فاعليته وسعره معا إضافة عظيمة للمريض المصرى.

الدكتور أشرف عمر، أستاذ الكبد فى جامعة القاهرة، سكرتير الجمعية المصرية لسرطان الكبد، يرى من جانبه أن وزارة الصحة مسؤولة عن تقييم العلاج، وأنها إما سمحت بتداوله فذلك يعنى أنه آمن ومفيد لمصلحة المريض، أو أنه فى بعض الأحيان يتم تسجيل أدوية معتمدة من الوكالة الأمريكية للغذاء والدواء «FDA» مع مرور الوقت حيث يثبت عدم فاعليتها أو لا يثبت، على الرغم من تسجيلها فى الولايات المتحدة الأمريكية على سبيل المثال، مشيراً إلى أنه شخصياً فى طريقه للحصول على نتائج نهائية تحدد مدى فاعلية عقار «رايفيرون ريتارد»، وهو ما يحتاج إلى وقت لا يقل عن سنتين أو ثلاث، على أن يتم تجريبه على عينة كبيرة من المرضى يمكن وضع إحصائيات ومؤشرات بناءً عليها، تحدد مدى الاستجابة والأعراض الجانبية لاستخدامه، على حد قوله. مطالباً بتكثيف حملات التوعية بالفيروسات الكبدية وطرق الوقاية منها، نظراً لمساهمة هذه الحملات فى تقليل مخاطر الإصابة بالمرض.

وفى القاهرة التقت «المصرى اليوم» الدكتور وفيق البرديسى، رئيس مجلس إدارة شركة «مينا فارم» للإجابة عن التساؤلات التى طُرحت على هامش المؤتمر السنوى الـ٦٠ للجمعية الأمريكية لدراسات أمراض الكبد، وأوضح أن الشركة تعاقدت عام ١٩٩٧ مع شركة «راين بايوتك» الألمانية- وهى ليست شركة أدوية- إنما إحدى الشركات الرائدة فى مجال الأبحاث والهندسة الوراثية، وتم التعاقد على مشروع بحثى مشترك، وهو ما تكلف ملايين الجنيهات- على حد قوله- وتم توقيع العقد وتوثيقه فى السفارة المصرية بألمانيا، وفى وزارة الصحة بمصر تمهيداً لإنتاج «الإنترفيرون المصرى الألمانى».

وتابع البرديسى: «نظراً لأن التكلفة كانت عالية للغاية عرضنا على (راين بايوتك) أن ندفع جزءاً ويدخل الجانب الألمانى شريكاً معنا.. وتم تأسيس شركة (راين مينا فارم)، وأُجريت عملية الإنتاج والتطوير على مدار سنتين ونصف السنة، بالتعاون مع مجموعة بحثية من هولندا، توصلت إلى منتج ثابت ومستقر معملياً، ليس تقليداً أعمى للإنترفيرون الذى تنتجه شركتا روش وشيرينج العالميتان،

وحول خطوات تسجيل الدواء، قال البرديسى: «تقدمنا بأوراق تسجيل الدواء، وعُرضت على ٧ لجان، وطلبوا تعديلات أرسلناها إلى الشركة الألمانية وتم تنفيذها، كما أجرينا تجارب إكلينيكية وخرجت نتائج مبشرة جداً.. وتم تسجيل الدواء فى مصر تحت اسم شركة وطنية كمستحضر مصرى لا علاقة له بألمانيا إلا من حيث التقنية،

أما حقوق المعرفة والتسجيل فهى مقصورة على شركة مينا فارم، ولم يتم الادعاء فى أى مرحلة بأن المستحضر مسجل فى ألمانيا، حيث إن ذلك يتطلب وثائق تفيد بذلك تقدم لوزارة الصحة المصرية، ولذلك فإن إثارة موضوع أن الدواء مسجل أو غير مسجل فى ألمانيا غرضها الشوشرة والانتقاص من نجاح المستحضر الذى أشادت به جمعيات حقوق الإنسان، ومنها جمعية فى أمريكا وصفت هذا النموذج المصرى للتعاون مع بلاد متقدمة لتوفير أدوية ذات تقنية عالية وأسعار مناسبة للفقراء بأنه نموذج يجب أن تحتذيه جميع البلاد النامية»،

وأضاف: «وما يثار الآن بصدد فاعلية المستحضر وما نشر فى المجلات العلمية العالمية بعد نجاحه فى السوق يتزامن مع قرار وزارة الصحة بإدراجه فى التأمين الصحى ليس بمحض الصدفة ولكنه بالتأكيد بفعل فاعل ويتعارض القرار مع مصلحته».

وتابع: صدر قرار بواسطة لجنة التسعير التابعة لوزارة الصحة بإعفائه من الضرائب، وبأن يأخذ الصيدلى ربحاً ١٠% فقط من بيع الدواء لأنه حيوى، على الرغم من أن القانون ينص على أن الصيدلى يحصل على نسبة ٢٠% من بيع الأدوية المصنعة محلياً، كما أسسنا مصنعاً لإنتاج الدواء بالتعاون بين خبراء ألمان ومصريين.. ويعتبر الأول والأحدث فى مصر والشرق الأوسط وأفريقيا.

وأكد البرديسى أن العديد من الدراسات العلمية والإكلينيكية جارية فى مصر وأوروبا ونتائجها مبشرة ومؤكدة، كما نشرت مجلات علمية متخصصة، ومع زيادة الاستهلاك الملحوظ حالياً الذى يؤكد ترحيب المجتمع الطبى بالدواء سينعكس ذلك على تكلفة الإنتاج ولن نتردد فى أن نخفض السعر دون حد أدنى.. فنحن أصحاب القرار هنا فى القاهرة لصالح الشركة والمواطن معا.. وليس لصالح الشركة فقط.

وأشار الدكتور أنور نصر، مدير المكتب العلمى بالشركة، إلى أن العقار موجود فى الأسواق منذ أكثر من ٣ سنوات، وبسعر ٣٧٠ جنيهاً، وأجريت عليه أبحاث متعددة بدأت بـ٥٠ مريضاً فى قصر العينى ثم وصلت إلى ١٠٠ والآن فى إطار ١٣٠٠ مريض، وأضاف: «هناك ٢٠٠ طبيب وأستاذ كبد متخصص يكتبون الدواء فى عياداتهم حتى تاريخه، كما هو مثبت فى الروشتات،

كما تعاقدت وزارة الصحة مع الشركة، بعد قرار من اللجنة القومية لمكافحة الفيروسات الكبدية، المكونة من أطباء هم أباطرة الكبد فى مصر، لأن العقار أجريت عليه تجارب ونُشر عنه فى الخارج، وبدأ استخدامه فى مستشفى أحمد ماهر التعليمى، إضافة إلى جميع المستشفيات الجامعية والهيئات الحكومية والمستشفيات الخاصة التى تستعمله منذ سنين بعد أن وثقت فى فاعليته.

وتابع: «ومع زيادة الاستهلاك تم لقاء بين مسؤولى الشركة، والدكتور سعيد راتب، رئيس هيئة التأمين الصحى، عرضنا خلاله تخفيض سعر الدواء للهيئة ليصل إلى ٢٢٥ جنيهاً، علماً بأن السعر للمستحضر الذى يوازى ربع المستورد قد ساهمت فيه مجهودات وزارة الصحة للضغط على الشركات العالمية لتخفيض سعر الإنترفيرون من ١٤٠٠ جنيه للحقنة إلى ٤٨٠ جنيهاً، ولنا أن نتصور حجم الاستغلال والأرباح التى حققتها هذه الشركات على مدار سنوات، ولم تجد الشركات العالمية ما تقوله نظراً لفاعلية الدواء من الناحية التحليلية على الأقل.. وكان بإمكانهم استخدام معاملهم الضخمة فى تحليل الدواء ولكنهم لم يثبتوا شيئاًً».

وأوضح د. نصر أن نتائج العقار تتراوح بين ٤٥ و٦٠% حتى الآن..وستُعلن قريباً النتائج بشكل أكثر تفصيلاً.. كما أن العقار نشر عنه عدة أبحاث فى كبرى المجلات العالمية «مجلة أمراض الهضم- الكبد- ٢٠٠٩»..

كما صنفه أحد أكبر علماء الإنترفيرون البروفيسور أنتونى ميجر من بين ثلاثة مستحضرات طويلة المفعول فقط فى العالم، إضافة إلى أبحاث ٣٠ أستاذاً مصرياً من جامعات القاهرة وعين شمس والأزهر والإسكندرية وطنطا وأسيوط ومعهد الكبد، مؤكدة فاعلية المستحضر والأعراض الجانبية المحدودة، خاصة المؤثرة على خصائص الدم، إضافة إلى أبحاث ودراسات أخرى نُشرت فى مجلتى «الدم»، و«بحوث الإنترفيرون» العالميتين.

من جانبه أكد الدكتور جمال عصمت، أستاذ أمراض الكبد بجامعة القاهرة، عضو اللجنة القومية لمكافحة الفيروسات الكبدية، أن وجود عقار «رايفيرون ريتارد» المصرى تسبب فى الضغط على الشركات الأجنبية المنتجة للإنترفيرون، مما دفعها لتخفيض سعره، وطرح أقراص الريبافايرين مجاناً، وساعدت الشركات فى حملات التوعية حول المرض وفى ظنى أن ما تردده بعض الشركات الأجنبية عن الدواء يدخل فى إطار الحرب ضد صناعة الدواء المصرية.

وأشار إلى أن دراسات أخرى أجريت أكدت فاعلية العقار المصرى، منها دراسة أجريت فى المعهد القومى للكبد. وأوضحت أن فاعليته متقاربة تماماً مع الأدوية الأخرى، ماعدا حالات التليف الزائد.

ووفقاً لعصمت فإن الدكتور حاتم الجبلى، وزير الصحة الحالى، وبحكم معرفته بقضية الالتهاب الكبدى فى مصر، ، وحصل على تخفيض من الشركتين المنتجتين للدواء ليقل سعر العبوة من ١٥٠٠ إلى ٤٠٠ جنيه وأن وجود العقار المصرى دفع الشركات الأجنبية إلى تخفيض أسعارها.

GSK signs DMD deal with Prosensa

GSK signs DMD deal with Prosensa

GlaxoSmithKline is linking up with Prosensa to develop drugs for Duchenne muscular dystrophy, an agreement that could be worth £428 million to the Dutch biopharmaceutical company.

Under the terms of the deal, GSK has acquired the rights to develop and sell Prosensa’s lead compound, PRO051, an RNA modulating therapeutic which acts by skipping exon 51 of the dystrophin gene. Three other RNA-based compounds, which seek to treat “specific, but different, subpopulations of patients suffering from DMD,” are covered by the agreement.

Cashwise, GSK is making an upfront payment of £16 million and Prosensa is eligible to receive up to £412 million in milestones if all four compounds are successfully developed. The Leiden-headquartere d firm is also entitled to double-digit royalties on product sales and will retain certain commercial participatory rights.

DMD is a severely debilitating neuromuscular disease that affects one in 3,500 newborn boys. Patients suffer from progressive loss of muscle strength due to the absence of the protein dystrophin, often making them wheelchair-bound before the age of 12, and most die in early adulthood due to respiratory and cardiac failure.

Hans Schikan, Prosensa’s chief executive, said that the alliance will not only speed-up the further development of PRO051, “but will also accelerate the progress of our complementary DMD therapeutics, allowing us to reach a broader patient population”. The deal comes a month after GSK’s R&D chief Moncef Slaoui revealed that the that company was looking to expand into the orphan drugs market.

Meantime, GSK has entered into a partnership with the UK Government, the Wellcome Trust and the East of England Development Agency to develop a biotechnology science park located at the company’s site in Stevenage . Full details in tomorrow’s PharmaTimes UK News elert.

Boehringer gets OK to sell extended-release Parkinson's drug

Boehringer Ingelheim has received the go-ahead in Europe to sell a once-daily version of its Parkinson’s disease drug Mirapexin.

The European Commission has granted marketing approval for prolonged-release Mirapexin/Sifrol( pramipexole) , a once-daily tablet for the treatment of early and advanced idiopathic Parkinson's disease. The approval was based on data showing that the new formulation can offer an efficacy and safety profile comparable to the immediate release tablet which is taken three times daily.

Boehringer noted that a further trial has shown that patients already taking Mirapexin may easily be switched overnight to the prolonged-release tablet, at the same daily dose. The new formulation causes less frequent fluctuations in the plasma concentration over 24 hours compared to the three-times daily drug, “an important aspect for physicians when choosing the best treatment option for their patient," said the company’s senior vice president of medicine, Manfred Haehl.

Boehringer quoted Mary Baker, president of the European Federation of Neurological Associations, as saying that most people with Parkinson's disease take many different pills on a daily basis, to manage their symptoms and other concomitant conditions. “Being able to reduce their pill burden without foregoing the effectiveness…can improve patient adherence to their treatment regimen," she added.

The extended-release formulation of pramipexole is currently under review with the US Food and Drug Administration.

Abbott beats forecasts as Humira sales soar again

Abbott Laboratories has posted a strong set of financials for the third quarter, helped by a one-time gain and the continuing revenue rise for Humira.

Net income leapt 36.5% to $1.48 billion, due in part to a $287 million gain from a settlement with Medtronic concerning stent technology. Group sales were up 3.5% to $7.76 billion and adjusting for the negative effect of foreign exchange, the increase would have been 8.4%.

Worldwide pharmaceutical sales slipped 1.6% to $4.06 billion, again due to currency effects, but the anti-inflammatory blockbuster Humira (adalimumab) , which is approved for indications covering rheumatoid arthritis, Crohn’s disease and psoriasis, soared once more. Sales of the drug shot up 23.8% to $1.49 billion and Abbott said that underlying demand for Humira remains strong across all three indications.

As for the company’s lipids franchise, TriCor (fenofibrate) and the recently-approved TriLipix (fenofibric acid) contributed $330 million, down 1.1%, while Niaspan (niacin) brought in $215 million, a rise of 10.6%. Of Abbott’s other products, sales of Depakote (divalproex sodium) for manic episodes, migraines and several types of epileptic disorders, were battered by generic competition and fell 70.9% to $92 million.

The prostate cancer therapy Lupron (leuprolide) fell 11.5% to $195 million, while revenues from the HIV drug Kaletra (lopinavir/ritonavi r) were down 8.7% to $353 million. Sales of the hypothyroid medication Synthroid (levothyroxine) were up 3.7% to $134 million.

Miles White, Abbott chief executive, said that the firm is performing well, having generated higher-than- expected earnings growth in the third quarter. He noted that the quarter also saw a series acquisitions, including the purchase of Solvay Pharmaceuticals. These deals “add to our diverse mix of global businesses, with new technologies, established products and emerging market infrastructure that will help us deliver sustainable industry-leading growth”, he added.

Roche has posted a strong set of sales figures for the third quarter this morning, with turnover rising 14% (excluding currency effects) to 12.39 billion Swiss francs, boosted by a huge rise in Tamiflu revenue.

Sales at the Swiss major’s pharmaceuticals division reached 9.93 billion francs, a rise of 15%. As usual, the oncology franchise performed well, with Avastin (bevacizumab) recording an increase of 21% to 1.59 billion francs, while MabThera/Rituxan (rituximab) grew 7% to 1.50 billion francs.

Herceptin (trastuzumab) was up 8% to 1.33 billion francs, while Tarceva (erlotinib) sales grew 11% to 319 million francs. Xeloda (capecitabine) turnover was up 11% to 326 million francs.

The most impressive performance, however, came from the influenza jab Tamiflu (oseltamivir) , sales of which leapt to 994 million francs from 101 million francs. The rise was due to the rapid worldwide spread of swine flu and Roche now expects full-year sales of Tamiflu of around 2.7 billion francs (up from a previous estimate of 2.0 billion francs) and about 700 million francs (up from 400 million francs) in 2010.

Of Roche’s other products, sales in the USA of Lucentis (ranibizumab) , for wet age-related macular degeneration, increased 21% to 296 million francs, while the hepatitis C treatment Pegasys (peginterferon alfa) was up 13% to 438 million francs. On the negative side, the transplantation product CellCept (mycophenolate mofetil) fell 26% to 362 million francs, while combined sales of Roche’s NeoRecormon and its Chugai unit’s Epogin (epoetin beta), for anaemia, declined 7% to 391 million francs.

Chief executive Severin Schwan said that sales by both pharmaceuticals and its diagnostics divisions “are significantly outgrowing their respective markets” and “based on this performance, we expect another very good full-year result”. He added that “I’m particularly pleased with the excellent progress we’re making in joining forces with Genentech”, which Roche fully acquired in March for $46.8 billion.

Roche has raised its outlook and now expects at least high single-digit sales growth for its pharmaceuticals division in 2009.

NEW YORK: Pfizer announced on Thursday it completed its acquisition of pharmaceutical rival Wyeth, a 68 billion dollar deal that cements its positions as the world's biggest drug manufacturer.

Pfizer, the maker of Viagra for erectile dysfunction and Lipitor for cholesterol, got final regulatory approval for the deal on Wednesday from US and Canadian authorities.

AstraZeneca has been boosted by the news that Japanese regulators have given the go-ahead to the company’s asthma drug Symbicort Turbuhaler.

Japan’s Ministry of Health, Labor and Welfare has approved Symbicort (budesonide/ formoterol) for the maintenance treatment of bronchial asthma in patients aged 16 and over when a combination therapy of an inhaled steroid and a long-acting beta-2 agonist is necessary. The approval is based on data from eight studies carried out in Japan .

Bruno Angelici, executive vice president for international sales and marketing at AstraZeneca, said that the approval will provide “a new treatment option for the approximately five million adults living with asthma in Japan ”. It will be co-promoted there by Astellas Pharma, which agreed to make an initial payment of 3.00 billion yen (about $31.5 million) for the rights in August. It may also pay up to 5.5 billion yen more, depending on sales and approval of an additional indication.

Symbicort is already a big-seller in Europe and the USA and second-quarter sales increased 24% to $551 million.
